1) Change of Ownership
In order for CNS to process a change of ownership for an existing CNS endorsed agent CNS must be formally notified in writing.
- Read the CNS regulation on Changes of Ownership, Name, Location (pdf)
- Fill out the Notice of Change Form (pdf)
Additional documents to be submitted:
- Notarized original current financial statement
- 3 months of bank statements prior to the date of the financial statement.
And, if applicable:
- Change in Name: certified copy of the Amendment to the Articles of Incorporation.
- Merger: certificate of Merger reflecting the surviving company name and which companies were merged together and filed with the proper state authority.
Fee:
- The fee of $800 must paid by check.
All this material and the check must be sent to
CNS mailing address.
2) Change of Name
In order to process a change of name, please follow these instructions:
- Read the CNS regulation on Changes of Ownership, Name, Location (pdf)
And submit the following documents:
- Amendment to the articles of incorporation filed with the proper State Authority reflecting that the name of the company has changed.
- Copy of a bank statement reflecting the new name of the company, or a letter from your bank reflecting that they have been advised of the change of corporate name.
In the event that a trade name will be added, please submit:
- Copy of the registration for the fictitious business or assumed name filed with proper state authority
- Copy of a bank statement reflecting the new trade name; or, a letter from your bank stating that they are aware of the addition of the trade name.
Fees
- The Change of Name or additional Trade Name fee amounts to $250, to be paid by check.
